Here are a few important things to know:
ALL PATIENTS:
NEW PATIENTS:
- We require a $100.00 deposit for holding new patient appointment time slots. This fee will be used toward your visit when you come in. We will contact you to collect your deposit once we receive your appointment request.
- We have a 48 business hour cancellation policy for New Patient visits. If you cancel under this time frame, we will retain your deposit.
- Being prepared for your new patient visit will ensure you receive the highest quality care:
- Complete your intake forms at least 48 hours before your appointment
- Bring bottles or labels of current supplements and medications to your appointment so your doctor can review them
- Upload and *share* any lab work you've had done in the last 1-2 years to your Charm patient portal 48 hours before your appointment
RETURN PATIENTS:
- For returning patients, we have a 24 business hour notice of cancellation for all follow-up visits. No-shows and cancellations that occur within less than 24 business hours before your appointment time will be billed at $75 to your credit card on file.
- If you need to reschedule or cancel your visit, you may do so online, by phone, or in person any time more than 24 hours before your visit at no cost to you.
